Goshen Family Chiropractic is now accepting new patients. Setting up your New Patient Consultation is as easy as 1, 2, 3!
1. Call the office at 610-344-0384 to schedule a 30 minute consultation. This appointment will include an exam and any necessary x-rays.
2. Visit our website to print and fill out the New Patient forms. There are three (3) forms to fill out New Patient Form, Consent & HIPPA Form, and Functional Rating Index.
3. Arrive to your appointment at the scheduled time and please wear a face mask or covering. We will have hand sanitizer as well as soap and water for you to wash your hands. Your health and safety as well as that of our staff is of the utmost importance to us.
